3/29/2023 EnchantaVaughn Approved Behavior: Client was present when QMHP entered the home. Client exhibited great functioning and well-being. Client appeared to be in a good mood and ready to talk. Client communicated that he was feeling good about himself since he woke up this morning. QMHPC engaged mother in a discussion to determine behaviors since the last session. Mother reports she struggles with Client not completing chores in the home and when he does she tries to reward him. Behavior: Client was excited and communicated he was ready for his therapy activity. Client struggled with being seated during the second portion of the session as he was leaving the room to see what his younger sibling was doing. He was redirected for his behavior and once redirected he was able to regain focus on the session. Intervention: QMHP provided simple praises and encouragement to continue the positive behaviors Client was exhibiting. QMHP determined Client was on task and did not require redirection. QMHP provided positive gestures to encourage Client on his positive performance. QMHP supported Client displaying positive social interaction in the home. QMHP implemented positive reward system regarding Client behaviors and choices. QMHP recognized Client for his hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. Intervention: QMHP engaged Client in a role reversal exercise and required Client to think about his goals. QMHP processed with Client about what he learned from activity after the review. QMHP utilized empathetic listening skills as Client expressed his understanding of ADHD. QMHP praised Client for his cooperation and staying seated during the role reversal exercise. QMHP explored Clientâs responses and responded accordingly. QMHP encouraged Client to continue being open with his emotions and donât be afraid to share his feelings even if they are different from someone elseâs. QMHP continued to support Client with making healthy decisions in the home. Response: Client transitions well between one on one discussion and family discussions, utilizing the correct voice volume and displaying positive body language the entire time. Client exhibits good attention skills and remained focused during the session. Client did a good job ignoring distractions and maintaining appropriate behavior in the session. Client was supported for using positive techniques for obtaining attention and confirmation of caring. Client continued to interact in a positive manner with his mother, siblings, and counselor. Mother praised Client for being on his best behavior. Mother agreed to start focusing and acknowledging Clientâs positive behaviors. Response: Client displayed management of individualized behaviors in the home. Client implemented good communication skills by listening attentively and closely. Client displayed full participation during family activity. It was noted that the interventions were successful. Client appeared to have a clear knowledge of the interventions. Client has not overreacted with anger to minor frustrations or irritants. Client understood his expectations and what ways he could utilize the session to best fit his learning style. Client responded well to QMHP and appeared thankful for the consistent help and support. Client was able to stay motivated and enjoy the rest of his day.
